I, the undersigned parent/guardian, hereby give permission for my minor child to participate in personal training sessions at Fitness Lounge NYC LLC, Fitness Lounge One LLC, Fitness Lounge Two LLC, aka Fitness Lounge. I acknowledge and understand that participating in physical activities involves inherent risks, including but not limited to muscle strain, injury, and other physical conditions. I assume full responsibility for any risks or injuries that may occur as a result of the minor’s participation in these activities.

I, the undersigned, do hereby release and hold harmless Fitness Lounge NYC LLC, Fitness Lounge One LLC, Fitness Lounge Two LLC, its employees, contractors, and agents from any and all liability, claims, demands, or causes of action arising from the minor’s participation in personal training sessions. This release includes, but is not limited to, any injury, illness, or damage to property sustained during or as a result of the minor’s participation.
